Cancellation Policy
We
strive
to meet
all of
our patient's
appointments
needs.
In order
to schedule
and see
patients
in a timely
manner,
it is
important
for all
patients
to keep
their
scheduled
appointments
and to
arrive
promptly.
Please
contact
the office
immediately
if there
is a need
to cancel
an appointment.
This will
allow
other
patients
who wish
to be
seen to
schedule
their
needed
appointments
in that
time slot.
In
the even
that an
appointment
is cancelled
with less
than 48
hours
notice,
or if
a patient
does not
show up
for an
appointment,
or if
a patient
is 30
minutes
late,
the patient's
next appointment
will be
scheduled
on a work-in
basis
only.
The doctor
will see
work-in
patients
if possible
between
scheduled
appointments.
Those
patients
with scheduled
appointments
that arrive
promptly
will be
seen first.
There
is a $25.00
charge
for missed/no
show appointments
and for
cancellations
made less
than 48
hours
(2 business
days)
before
a scheduled
appointment.
This includes
Trigger
Point
Injections,
Physical
Therapy,
Rehab
Counseling
and failure
to obtain
authorization
for a
referral.
Exceptions
will be
made for
emergencies
such as
hospitalizations
(documentation
may be
required).
This charge
is NOT
payable
by your
insurance
plan and
will be
payable
at the
next scheduled
appointment.
This
policy
is in
order
for those
patients
that do
keep their
appointments
and do
arrive
promptly
to be
able to
be seen
in a timely
manner.
Please
note:
Most medications
may only
be refilled
at scheduled
appointment
times
and will
NOT be
refilled
over the
telephone.
